Please collect and prepare and send the following documents:

1. You must send your valid US Passport with at least 6 months of validity left and one blank visa page (blank endorsement pages in your passport are not acceptable).

2. 2 Passport Photos. They must be 2x2, color or black and white with a plain light or white background with a full front view of face.

3. Visa Application Form (signed and completed)

4. Please Fill Out the American Passport Express Customer Information Form

5. Processing Fees:

One Money Order made payable to the Consulate General of India (see table below) Do not send personal or company checks. Include your name and date of birth on the money order. Do not include your address on the money order.

One check or money order made payable to American Passport Express for $75

Include an overnight return fee of $ 17 for FedEx Priority Overnight which can be added to the American Passport Express check OR you may include a prepaid FedEx airbill for the return shipping.

When you have the numbered list of items above, and your FedEx airbill in front of you call American Passport Express (1-800-841-6778) for the Address to Send your package.

You must call for the shipping address by 9:00 Pm EST M-Th and 8:00 Pm EST Friday.
