How to Order

T-Shirts, etc.

1- Print off order form.Youth order form or the adult order form. Completely fill out form and attach a check to "Married to Medicine" or fill out the credit card information.

2- If you are at the school, give order form (with payment) to Marcia Cusic. DO NOT leave order forms in Sean’s box! We want to keep your information (especially your credit card information) safe, so please don't leave forms in any boxes!

3- If you are not at the school, you can place a credit card order by calling Alicia Wood at (402) 238- 7519 or email her at mommywood314@yahoo.com. 

3- If you are out of state and would like to order a shirt, you can mail us order forms (with credit card info filled out or with a check enclosed). Please include an email address where we can reach you with all mail orders.
