Re:       Faculty Reappointment

Dear Dr. «Last_Name»:

The Department of Medicine appreciates and is grateful for the many activities in which you, as a member of the Contributed Service Faculty, have participated. At this time, according to the regulations for Creighton University School of Medicine reappointment of your current position as «Primary_Rank» is required. If you are involved with teaching medical students and/or residents, you are required to have a faculty appointment. I am pleased to offer the renewal of your Contributed Service Faculty appointment. However, the following paperwork needs to be completed before we finalize this reappointment.

1.                 A current curriculum vitae;

2.                 A copy of your current medical license;

3.                 A copy of your Federal and State DEA certificates;

4.                 A copy of your malpractice insurance;

5.                 Complete the attached form indicating the specific activities you are planning to or are currently participating:

a.                   Supervise Creighton medicine students, residents or fellows in outpatient offices

b.                  Regular participation in Creighton Medicine journal clubs and/or case conferences

c.                   Creighton Medicine Grand Rounds or noon conference presentation

d.                  Supervise Creighton Medicine students, residents or fellows in the care of inpatients

e.                   Participate in research with Creighton Medicine faculty

f.                    Other contributions:
